I give permission for an agent of Boxer Rescue of Albuquerque, Inc., to type my name into this form, and that that shall be the same as me signing this legal document.

I testify that I am financially and physically able to care for a Boxer. I understand that proper food, veterinary care, bedding, toys, crate and so on can be costly, and that I am able to meet these costs. I further testify that I am physically fit to provide my dog with the necessary type and level of activity.

I agree to notify Boxer Rescue of Albuquerque if my dog is missing.

I agree to return my dog to Boxer Rescue of Albuquerque if I am no longer able to care for it.

I agree to allow an agent of Boxer Rescue of Albuquerque to visit my home before and after adoption.

I understand that if the information herein is found to be false, my application can be refused, or adopted dog will be relinquished to Boxer Rescue of Albuquerque, Inc., without refund of adoption fee.
